Pre-operative registration

The Bordeaux-Mérignac Sport Clinic provides you with the free and secure APPFINE portal to carry out pre-admission formalities online.

As soon as your surgery date is set, you receive a login link by email and personalized access code by SMS.

If you do not have an email address, you can download the documents (below) or request a paper file at the clinic reception, fill out and return by mail with the following documents :

• Copy of ID, Passeport or Résidence Permit.
• Up-to-date social security certificate (to be requested from your CPAM)
• Supplementary health insurance card (both sides)
• Consent for surgery
• Trust person appointment
• Declaration of Work Injury (if applicable)

additional documents for minors :

• Parental consent MUST be signed by both parents
• Copy of BOTH parents ID
• Copy of family booklet
